Momentum Long COVID Support – Terms & Conditions

1. Overview

• Momentum Therapy previously delivered an 8-week Long COVID Program funded by Capital Health Network. While the government-funded program has now concluded, we recognise that many people continue to experience ongoing symptoms and challenges following COVID-19.

• We are continuing to offer support for people living with Long COVID through an individualised approach designed around your needs, goals and current capacity.

• The Momentum 8-Week Long COVID Management Program was free for the first 50 eligible participants who signed up before June 30, 2025 and then was extended until the 31st Dec 2025. There will be no further extensions for free programs in 2026.

• No referral is needed to join.

• Note: There will be a cost to any participants that sign up

 

2. Eligibility Criteria

• Participants must complete a pre-screening assessment to determine their suitability for the program.

• Entry to the program is subject to clinical approval based on this assessment.

• Our team will guide you through a short pre-screening process to assess suitability.

• If you qualify, we will book your initial consultation.
